Based on the requirements of Prevention & Control Policy for Non-Communicable Disease and related Risk Factors in the Emirate of Dubai, which was approved by the Dubai Executive Council in year 2018, thus Primary Healthcare Sector - DHA would like to invite you to participate in an online survey and provide information and data related to non-communicable disease services at your facility.

The main objective of the questionnaire is to collect government indicators for implementing the policy and improving related services in Dubai.

Accordingly, we kindly ask that you submit your results by 31 September 2019
